About The Position

The Memory Care Support Partner works in the memory care neighborhood at Vitalia Montrose in Copley, Ohio. The role focuses on building empathetic connections with residents, learning about their interests and abilities, and collaborating on engagement plans. The partner helps create a supportive daily environment, assists residents with daily life from waking through bedtime, documents care and changes in condition, and helps orient team members assigned to memory care neighborhoods. This is a full-time position on a 6:00 a.m. to 6:00 p.m. schedule. The schedule alternates: Week 1 is Tuesday, Thursday, and Friday; Week 2 is Monday, Saturday, and Sunday. Pay is $18.25–$27.25 per hour.

Requirements

Be at least 18 years of age.
Have a high school diploma or equivalent.
Be able to read, write, understand, and communicate in English with residents.
Have a positive, energetic attitude and a strong interest in working with residents.
Have computer skills sufficient to print documents and enter resident information.
Be able to stand, walk, bend, kneel, and stoop throughout the day.
Be able to frequently lift and/or move items weighing up to 50 pounds.
Be able to assist residents with sitting, standing, walking, and after a fall.
Receive criminal clearance.

Responsibilities

Create empathetic human connections with residents, including during vulnerable moments.
Observe residents to learn their interests, abilities, and individual characteristics.
Collaborate on appropriate engagement plans and implement strategies patiently.
Prepare and arrange spaces and residents’ rooms to support daily activities and routines.
Partner with residents in the facets of daily life, from waking through bedtime.
Document resident care and changes in condition.
Assist with orienting team members assigned to memory care neighborhoods.
Assist residents with sitting, standing, and walking, and assist persons after a fall.
