About The Position

The Medication Reconciliation Assistant (MRA) works within the health system under the supervision of a pharmacist to collect, validate, and accurately document the best possible medication list for patients. This list includes the correct drug name, strength, route, and administration frequency for all current prescription and over-the-counter medications. Information may be gathered by interviewing patients and their families or caregivers, contacting external medical providers and facilities or outpatient and mail-order pharmacies, and reviewing electronic medical records from prior hospital encounters. The MRA documents medication information in the electronic medical record for the current hospital encounter and communicates corrections or additions to medical providers for review and appropriate action. The role may also involve obtaining, adding, or correcting medication allergy information, including the medication name and reaction type. The assistant responds to providers' and patients' needs, manages and prioritizes tasks to support timely and safe care, and participates in quality improvement, staff education, training, committees, and initiatives related to the Medication Reconciliation Program. The MRA serves as a liaison between the Medication Reconciliation Team and hospital clinical staff and provides guidance to less experienced personnel. The MRA follows Emory Healthcare PLEDGE and its care transformational model, contributes to teamwork and continuous quality improvement, adapts to change, reports safety concerns, maintains professional knowledge, and completes required training and competency assessments. Other duties may be assigned.
