About The Position

The Medication Reconciliation Assistant (MRA) works within the health system under a pharmacist’s supervision to collect, verify, and document an accurate medication list for the current hospital encounter. The list includes the correct drug name, strength, route, and frequency for prescription and over-the-counter medications. Information may be collected by interviewing patients and their families or caregivers, contacting external medical providers and facilities or outpatient and mail-order pharmacies, and reviewing electronic records from prior hospital encounters. The MRA documents the information in the electronic medical record and communicates corrections or additions to medical providers for review and action. The MRA may also obtain, add, or correct medication allergy information, including the medication and type of reaction. The role responds to calls and messages from patients and medical providers, triages requests, and prioritizes work to support timely and safe patient care. It contributes to quality improvement projects, including collecting, analyzing, and presenting related data, and develops or presents educational materials to improve patient safety and quality of care. The MRA trains and orients new personnel, supports less experienced technicians, and serves as a liaison between the Medication Reconciliation Team and hospital clinical staff. The position also participates in committees, professional activities, and initiatives related to the Medication Reconciliation Program, completes required training and competency assessments, and performs other duties as assigned. The MRA follows Emory Healthcare’s PLEDGE and care transformational model, contributes to teamwork and continuous quality improvement, and reports safety concerns appropriately.
