About The Position

The Pharmacy Medication Reconciliation Technician improves medication safety by assisting pharmacists in collecting and documenting up-to-date medication lists for patients being admitted to the organization’s hospitals. The role helps optimize the medication reconciliation process and reduce medication errors resulting from inaccurate or incomplete information.

This position is in the Network Pharmacy Tech Float Pool, serving various locations in Phoenix, Arizona. It is PRN and works day hours from 6:00 a.m. to 2:30 p.m.

Requirements

• High school diploma or GED.
• More than 10 months of pharmacy technician experience, or completion of a formalized technician training program.
• Current Certified Pharmacy Technician (CPhT) certification/license through the Arizona State Board of Pharmacy.
• Current Pharmacy Technician Certification (CPhT) issued by the Pharmacy Technician Certification Board (PTCB).

Nice To Haves

• Completion of a formalized technician training program.
• Retail pharmacy experience and/or excellent customer service skills.
• Pharmacy Technician Diploma (LPT).

Responsibilities

• Obtain and verify medication histories in the electronic medical record accurately and promptly, in compliance with standard procedures and regulatory standards.
• Accurately document medication lists in the electronic health record, communicate with the verifying pharmacist, and promptly relay outstanding transition-of-care issues to the pharmacist and/or provider.
• Perform other duties as assigned.
