About The Position

This position is for an LPN in the Discharge Lounge, focusing on scheduling, charting, and patient care. The role requires an active New York LPN license and BLS certification. Experience in acute care within the last two years is necessary, along with proficiency in various clinical skills including vital signs monitoring, medication administration, wound care, and Foley catheter insertion. The LPN will manage their own patient assignments with RN support. The unit primarily handles medical patients with conditions such as COPD exacerbation, infections, failure to thrive, and social issues. Floating within skillset is required. The hospital, Rochester General Hospital, is a 528-bed acute care teaching facility recognized by Magnet and as an Advanced Thrombectomy Capable Stroke Center, affiliated with Cleveland Clinic and Roswell Park.

Requirements

• Active New York Single State LPN license
• BLS certification
• EPIC experience required
• Acute Care experience within the last 2 years
• Ability to take own patient assignment with RN support

• Experience with Telemetry monitoring
• Experience with 12-lead EKG
• Experience with wound care basics
• Experience with Foley catheter insertion

Responsibilities

• Take own patient assignment with the support of an RN
• Perform vital signs, pain assessment, intake/output monitoring
• Administer medications (oral, IM, SQ, NG/G-tube, ear and eye drops)
• Perform IV therapy with pumps
• Monitor blood glucose
• Perform wound care basics (simple dressings, JP/Hemovac drains)
• Assist with bathing and mouth care
• Perform Foley catheter insertion (male/female)
• Collect clean catch urine samples
• Communicate status changes to RN/MD
• Perform Telemetry monitoring
• Perform 12-lead EKG
• Manage oxygen administration via nasal cannula
• Manage feeding tubes
• Adhere to isolation precautions and physical restraints policies
• Perform specimen labeling
• Float within skillset as required
