About The Position
This is a travel, temporary assignment for a Licensed Practical Nurse (LPN) in a Long Term Care setting, specifically an Assisted Living Facility, located in Crivitz, Wisconsin. The assignment is managed by Healthforce Healthcare LLC and offers a flexible shift with guaranteed 36 hours per week. The contract duration is for several weeks, with a start date of 10/26/2026. The role involves delivering direct nursing care, administering medications and treatments, monitoring residents, and collaborating with an interdisciplinary team to ensure resident safety and well-being. Accurate documentation in the electronic health record and adherence to infection control protocols are essential.
Requirements
- Active LPN license (in good standing); state licensure requirements for practice apply.
- Current CPR/BLS certification.
- Ability to work in a long-term care/assisted living environment and adapt to flexible scheduling.
- Comfortable with charting, medications administration, and collaborating with a multidisciplinary team.
- Must be able to pass standard health screenings and background checks as part of the assignment process.

- Prior nursing experience in long-term care, assisted living, or a similar setting preferred.
Responsibilities
- Deliver direct nursing care to residents in a long-term care/assisted living setting, following individualized care plans.
- Administer medications and treatments per physician orders; monitor for side effects and document responses.
- Conduct routine assessments, monitor vital signs, and report changes to the care team.
- Collaborate with an interdisciplinary team to coordinate care, support activities of daily living, and ensure resident safety.
- Maintain accurate charts and documentation in the electronic health record; adhere to regulatory requirements and infection control protocols.
- Respond promptly to changes in condition, provide triage when needed, and assist with emergency procedures as trained.
