Requirements

Active LPN license.

Responsibilities

Provide direct nursing care to residents under the supervision of licensed staff, following the plan of care.
Administer medications and treatments as prescribed, documenting responses and changes.
Monitor vital signs, assess patient status, and report significant findings to the supervising nurse.
Assist residents with activities of daily living and mobility, promoting comfort, safety, and dignity.
Maintain accurate, timely documentation in charts and electronic systems; communicate changes to the care team.
Support infection control practices, safety protocols, and care coordination with RNs, physicians, and other staff.
