About The Position

This is a travel, temporary assignment for a Licensed Practical Nurse (LPN) in a Long Term Care setting in Lebanon, New Hampshire. The assignment is for 13 weeks, with guaranteed 36 hours per week. The shift is 3x12-hour days (07:00–19:30). This is a local assignment, requiring work on Wednesdays, Thursdays, and every other weekend.

Requirements

• Active Licensed Practical Nurse (LPN) license in good standing (New Hampshire) or eligibility to practice
• Graduation from an accredited practical nursing program
• Ability to perform duties in a long-term care setting and work 12-hour shifts
• Strong communication and teamwork skills

Responsibilities

• Administer prescribed medications and treatments according to physician orders and facility policy
• Monitor residents’ conditions, identify changes, and communicate findings to the care team
• Perform routine nursing assessments and document results accurately in the chart
• Assist residents with activities of daily living and personal care needs
• Maintain safety practices, infection control standards, and proper equipment use
• Collaborate with RNs, physicians, and other clinicians to develop and implement individualized care plans
