About The Position
Concierge Home Care is hiring a Home Health Licensed Practical Nurse (LPN) to provide one-on-one skilled nursing care in patients’ homes in Clay County, Florida. The role offers PRN or full-time/modified full-time scheduling, a pay-per-visit compensation model, and autonomy to create and manage a daily schedule. Territories are designed to minimize drive time through clustered scheduling, and consistent patient volume is provided. The LPN works under the supervision of a Registered Nurse, supporting patient assessments and care planning, administering ordered medications and treatments, educating patients and caregivers, monitoring patient conditions, and documenting care with AI-assisted tools and an EMR. The position includes structured onboarding, mentorship, clinical training, and career advancement opportunities. Compensation is $30–$35 per visit, with stated average full-time earnings of $55,000–$70,000 or more annually, quarterly bonus opportunities, and mileage reimbursement.
Requirements
- Active, unencumbered Florida Licensed Practical Nurse (LPN) license.
- Valid driver’s license, reliable transportation, and auto insurance.
- CPR certification.

Responsibilities
- Provide skilled nursing care under the supervision of a Registered Nurse.
