About The Position
Concierge Home Care is hiring a Home Health Licensed Practical Nurse (LPN) to provide one-on-one skilled nursing care in patients’ homes in Downtown and Arlington, within Duval County, Florida. Territories are designed to minimize drive time through clustered scheduling. The role offers a pay-per-visit model, consistent patient volume, and flexible scheduling, including PRN or modified full-time options. LPNs manage their daily schedules and work under the supervision of a Registered Nurse, without hospital shifts or 12-hour rotations. The position includes structured onboarding and mentorship, ongoing clinical training and development, free CEUs, and career advancement opportunities. Documentation is completed with an EMR and AI-assisted charting tools. Compensation is $30–$37 per visit, with stated average full-time earnings of $55,000–$70,000 or more annually, quarterly bonus opportunities, and mileage reimbursement.
Requirements
- Active, unencumbered Florida Licensed Practical Nurse (LPN) license.
- Valid driver’s license, reliable transportation, and auto insurance.
- CPR certification.

- Home health experience is preferred but not required.
Responsibilities
- Provide skilled nursing care under the supervision of a Registered Nurse.
- Perform patient assessments and assist with care planning.
- Administer medications and treatments as ordered.
- Educate patients and caregivers about disease management and care plans.
- Monitor patient condition and report changes to the supervising Registered Nurse.
- Collaborate with physicians and interdisciplinary teams.
