About The Position
Concierge Home Care is hiring a Home Health Licensed Practical Nurse (LPN) to provide one-on-one skilled nursing care in patients’ homes in Duval County, Florida, with coverage in Southside and the Beaches. Territories are designed to minimize drive time through clustered scheduling. The role offers schedule autonomy, PRN or modified full-time options, and a pay-per-visit compensation model. Consistent patient volume is provided, with base pay of $30–$37 per visit and stated average full-time earnings of $55,000–$70,000+ annually. Quarterly bonus opportunities and mileage reimbursement are also offered. The LPN provides care under the supervision of a Registered Nurse, performs patient assessments, assists with care planning, administers ordered medications and treatments, educates patients and caregivers, monitors patient conditions, and reports changes to the supervising RN. The role includes collaborating with physicians and interdisciplinary teams and documenting care using AI-assisted tools and the WellSky/Kinnser EMR. Requirements
- Active, unencumbered Florida Licensed Practical Nurse (LPN) license.
- Valid driver’s license, reliable transportation, and auto insurance.
- CPR certification.
