LPN Licensed Practical Nurse Home Health
About The Position
Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The Home Health Licensed Practical Nurse (LPN), also referred to as a Licensed Vocational Nurse (LVN), works under the supervision of a Registered Nurse to provide skilled nursing care to patients in their homes. The role involves coordinating with the RN and physician, educating patients and caregivers, monitoring patients, performing ordered procedures, and documenting care. Requirements
- Current LPN license specific to the state or states assigned for work.
- Current CPR certification.
- At least 1 year of LPN or LVN experience.
- Valid driver's license, reliable transportation, and liability insurance.
Responsibilities
- Verify the plan of care with the RN before each visit.
- Collaborate frequently with the physician and RN to ensure patient needs are met, and notify them as soon as possible when a patient's condition changes.
- Teach patients and caregivers self-care techniques.
- Check vital signs.
- Review all medications and perform medication reconciliation at each visit.
- Perform specialized procedures ordered by the physician, such as Foley catheter changes.
- Prepare equipment and materials for treatments using aseptic technique.
- Complete documentation accurately and on time.
