About The Position

Adoration Home Health and Hospice is seeking a dedicated Home Health Licensed Practical Nurse (LPN) to provide patient-centered care in patients’ homes in High Point, North Carolina, and the listed coverage areas of Guilford County, Kernersville, Randolph County, and Thomasville. The role provides one-to-one care according to each client’s plan of care, supporting their dignity and independence while working independently with team support. The schedule is PRN.

Requirements

• LPN/LPT/LVN degree from an accredited college of nursing.
• Current unrestricted registration and license in the state of practice and state of residence.
• Valid driver’s license, acceptable driving record, and proof of car insurance.
• Professional CPR certification.

Nice To Haves

• At least one year of nursing experience.
• Home health care experience.

Responsibilities

• Provide one-to-one care according to the client’s plan of care while maintaining the client’s dignity and independence.
• Administer medications, feedings, oxygen, and ostomy care as needed under the plan of care, and assess and report results.
• Accurately document nursing care, communications, progress, and outcomes related to established goals.
• Inform the physician, case manager, and family of changes in the client’s medical condition and needs.
• Coordinate or order supplies, medications, and feedings to ensure adequate amounts are available.
• Ensure patient needs are met in accordance with physician instructions.
