About The Position

Adoration Home Health is seeking a dedicated Home Health Licensed Practical Nurse to provide patient-centered care in patients’ homes. The position is based out of the Jasper, Alabama office, with primary coverage in Jefferson County and possible coverage in Walker and Winston counties. The schedule is full time, Monday through Friday, 8 a.m. to 5 p.m. The nurse provides one-to-one care under each client’s plan of care, supporting clients’ dignity and independence while working independently with team support.

Requirements

• LPN/LPT/LVN degree from an accredited college of nursing.
• Current unrestricted registration and license in the state of practice and state of residence.
• Valid driver’s license, acceptable driving record, and proof of car insurance.
• Professional CPR certification.

Nice To Haves

• One or more years of nursing experience preferred.
• Home health care experience preferred.

Responsibilities

• Provide one-to-one care according to each client’s plan of care while maintaining the client’s dignity and independence.
• Administer medications, feedings, oxygen, and ostomy care as needed, and assess and report results.
• Accurately document care provided, nursing actions, communications, progress, and outcomes related to established goals.
• Inform the physician, case manager, and family of changes in the client’s medical condition and needs.
• Coordinate or order supplies, medications, and feedings to maintain adequate availability.
• Ensure patient needs are met in accordance with physician instructions.
