About The Position

Phoenix Home Health is hiring a full-time Licensed Practical Nurse to provide one-on-one skilled nursing care in patients’ homes in Springfield, Missouri, and surrounding communities. Nurses deliver care according to physician orders and individualized care plans, assess patients and monitor changes in their condition, and educate patients and families to support recovery and independence at home. The role involves collaborating with RNs, therapy providers, physicians, and the interdisciplinary care team, while maintaining accurate documentation with AI-supported charting technology during patient visits. The schedule is Monday through Friday, 8:00 AM to 5:00 PM, with a weekend rotation approximately every four to six weeks. Phoenix offers clinical leadership support, orientation and training, paid mileage, and opportunities for growth. The listed pay range is $60,000–$75,000.

Requirements

Current Missouri or Compact LPN license.
Excellent assessment and communication skills.
Strong organizational and time management abilities.
Compassionate, patient-centered approach to care.
Valid driver’s license and reliable transportation.
