About The Position

Amedisys is hiring a full-time, day-shift Licensed Practical Nurse to provide home health care. The role supports patients in healing at home and involves coordinating with registered nurses and physicians, educating patients and caregivers, monitoring patient health, administering ordered procedures, and documenting care. Compensation is $25.00–$31.00; pay may vary by geographic market, job-related knowledge, skills, and experience.

Requirements

• One year of clinical LPN experience.
• Current LPN license specific to the state(s) assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Verify the plan of care with the RN before each visit.
• Collaborate frequently with the physician and RN to ensure patient needs are met, and promptly notify them of changes in a patient's condition.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform specialized procedures ordered by a physician, such as changing a Foley catheter.
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation accurately and in a timely manner.
