About The Position

Adoration Health is seeking a dedicated Home Health Licensed Practical Nurse to provide patient-centered care in Glasgow, Kentucky, covering Allen, Barren, and Hart Counties. The role involves providing one-to-one care in patients’ homes according to their plans of care, supporting their dignity and independence, and coordinating with families, case managers, physicians, and the care team. The position is part-time and offers flexible scheduling, independent work with team support, and opportunities for advancement.

Requirements

• LPN/LPT/LVN degree from an accredited college of nursing.
• Current unrestricted registration and license in the state of practice and state of residence.
• Valid driver’s license, acceptable driving record, and proof of car insurance.
• Professional CPR certification.

Nice To Haves

• At least one year of nursing experience preferred.
• Home Health care experience preferred.

Responsibilities

• Provide one-to-one care according to each client’s plan of care while maintaining the client’s dignity and independence.
• Administer medications, feedings, oxygen, and ostomy care as needed under the client’s plan of care, and assess the results.
• Accurately document care and nursing actions, communications, progress, and outcomes related to established goals.
• Inform the physician, case manager, and family of changes in the client’s medical condition or needs.
• Coordinate or order supplies, medications, and feedings to maintain adequate availability.
• Ensure patient needs are met in accordance with physician instructions.
