Licensed Practical Nurse (LPN) Home Health
About The Position
Phoenix Home Health is hiring a Licensed Practical Nurse to provide home health nursing care in Hutchinson, Kansas, and surrounding areas. The LPN serves as a link between patients and medical providers, conducting physical assessments and communicating with the care team to carry out individualized care plans based on physician orders and assessment findings. The role collaborates with nurses, Physical Therapists, Speech Therapists, and Occupational Therapists. Nurses complete charting on-site with support from Roger’s AI-powered assistance. The position is offered as a PRN opportunity and includes orientation and training tailored to the new hire. Requirements
- Kansas or Compact Licensed Practical Nursing license.
- Strong organizational and self-management skills.
- Strong and compassionate customer service skills.
- Valid driver’s license.
Responsibilities
- Assist in reviewing and revising the plan of care.
- Provide nursing care according to the physician’s plan of treatment and the individual plan of care, as authorized by the client’s payer source.
- Observe, record, and report reactions to treatment and changes in the client’s condition to appropriate personnel and/or the physician.
- Provide ongoing instruction to the client or family regarding the plan of treatment.
- Participate in education programs, quality assessment activities, nurses’ meetings, and other staff meetings as required.
- Collaborate with nurses, Physical Therapists, Speech Therapists, and Occupational Therapists.
