About The Position
The Home Health Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) works under the supervision of a Registered Nurse to provide skilled nursing care to patients in their homes. The role involves coordinating with the RN and physician, supporting patients and caregivers with self-care education, monitoring patient health, reviewing medications, and performing physician-ordered procedures. The position includes visit-based compensation for patient visits and hourly compensation for certain non-visit activities. Annual total cash compensation assumes full-time employment of 40 hours per week at full productivity and varies with factors including local labor markets, education, work experience, productivity, and performance. Requirements
- At least 1 year of LPN or LVN experience.
- Current LPN license specific to the state or states assigned for work.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
Responsibilities
- Verify the plan of care with the RN before each visit.
- Collaborate frequently with the physician and RN to ensure patient needs are met, and notify them as soon as possible when a patient's condition changes.
- Teach patients and caregivers self-care techniques.
- Check patients' vital signs.
- Review all medications and perform medication reconciliation at each visit.
- Perform specialized procedures ordered by the physician, such as Foley catheter changes.
- Prepare equipment and materials for treatments using aseptic technique.
- Complete documentation accurately and on time.
