About The Position

This is a travel Licensed Practical Nurse (LPN) position in Long Term Care located in Bala Cynwyd, PA. The contract is for 12 weeks, starting on 08/26/2026. The role involves delivering skilled LPN nursing care during home visits, with a focus on wound care management and documentation. The nurse will assess patient condition, monitor vital signs, administer medications, and implement care plans under supervision. Responsibilities also include performing wound care procedures, maintaining accurate electronic health records, educating patients and families, and coordinating with other healthcare providers.

Requirements

• Active Licensed Practical Nurse (LPN) license in Pennsylvania (or eligible for licensure in PA)
• Minimum 2 years of home care experience
• Proficiency in wound care required
• Ability to work in a home healthcare setting and cover patient visits in 19104 and 19139 zip codes
• Comply with agency policies, documentation standards, and patient safety guidelines

Responsibilities

• Deliver skilled LPN nursing care during home visits, with a focus on wound care management and documentation.
• Assess patient condition, monitor vital signs, administer prescribed medications, and implement plan of care under supervision.
• Perform wound care procedures, dressing changes, and routine clinical tasks in accordance with clinical policies.
• Maintain accurate and timely charting in the electronic medical record; communicate updates to the RN supervisor and care team.
• Educate patients and families on care plans, medication administration, safety, and self-management strategies.
• Coordinate with physicians, therapists, and other providers to ensure continuity of care and adherence to care plans.
