About The Position

This is a travel assignment for a Licensed Practical Nurse (LPN) in Long Term Care, located in Waterloo, NY. The assignment is for 26 weeks and begins on 09/01/2026. The role operates on a nights shift with a 5x8 rotation. The position is within a skilled nursing facility and requires flexibility to work weekends and holidays as needed. This is a temporary assignment.

Requirements

• Active NY LPN licensure
• BLS certification (through American Heart Association or RQI)
• Minimum 1 year of LPN experience, preferably in a long-term care setting
• Ability to work night shifts and weekends/holidays as scheduled; adapt to the 5x8 night rotation
• Competence in caring for adult and geriatric residents; strong collaboration with an Interdisciplinary Team
• Willingness to follow defined policies and procedures under RN supervision and maintain patient safety

Responsibilities

• Provide direct and indirect skilled nursing care for residents on the assigned unit under the supervision of a Registered Nurse.
• Collect data on residents’ physical and psychosocial conditions and communicate findings to the RN and Interdisciplinary Team.
• Contribute to development of an age-appropriate, culturally relevant plan of resident care reflecting nursing diagnoses and interventions; involve residents and families in the process.
• Recommend revisions to the plan of care after evaluating resident outcomes and refer to appropriate services to address all aspects of care.
• Maintain current knowledge and competency in Nursing Practice; participate in continuing education and in-service programs.
• Demonstrate leadership in the professional practice setting and collaborate with other nursing staff, physicians, and ancillary team members; provide mentoring to peers as needed.
