About The Position

This is a travel temporary assignment with Healthforce Healthcare LLC, designed to support ongoing resident care in a long-term care environment. The role involves providing direct nursing care to residents during the night shift, administering medications, monitoring resident status, assisting with daily living activities, and collaborating with the interdisciplinary team. Requirements

Active Licensed Practical Nurse license (state-specific requirements may apply)
Ability to work night shifts (3x12) and meet guaranteed 36 hours per week
Effective communication and teamwork skills; strong documentation habits
Willingness to work in a long-term care setting with residents
Prior nursing experience in long-term care or skilled nursing environments preferred
Ability to complete onboarding and credentialing as required by Healthforce Healthcare LLC

Responsibilities

Provide direct nursing care to residents during the night shift, including assessment and monitoring.
Administer prescribed medications and document administration accurately in the patient record.
Monitor resident status, vital signs, and changes in condition; report significant findings promptly to the charge nurse.
Assist residents with activities of daily living, hygiene, mobility, and personal care while maintaining safety and dignity.
Collaborate with the interdisciplinary team to implement and adjust individualized care plans.
Ensure strict adherence to infection control, safety, and facility policies; maintain accurate documentation and quality standards.
