About The Position
This is a travel assignment for a Licensed Practical Nurse in Long Term Care, located in Santa Fe, New Mexico. The assignment is for 8 weeks, with a start date of 08/20/2026. The role guarantees 36 hours per week, with 12-hour day shifts from 06:00 to 18:30. The setting is a long-term care nursing home. This position is hosted by Healthforce Healthcare LLC.
Requirements
- Active Licensed Practical Nurse license in New Mexico.
- Current CPR/BLS certification.
- Ability to work 12-hour day shifts in a long-term care environment and adapt to a travel assignment.
- Strong documentation, communication, and teamwork skills; comfortable using electronic health records.
Nice To Haves
- Prior long-term care experience is preferred but not required.
Responsibilities
- Administer prescribed medications and treatments within scope of practice and facility policy.
- Monitor residents’ health, perform routine assessments, document vital signs, and report changes to the supervising RN.
- Assist with activities of daily living, mobility support, hygiene, nutrition, and safety to promote residents’ comfort and well-being.
- Maintain accurate documentation in the electronic medical record; ensure HIPAA compliance and confidentiality.
- Collaborate with the interdisciplinary team to implement care plans and respond promptly to call lights and changing resident needs.
