About The Position

We are currently seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team at Home Health Services of Virginia. This full-time position is based in Richmond, VA and offers a stable Monday through Friday schedule from 8:00 AM to 3:00 PM, providing an excellent work-life balance. As an LPN with our organization, you will play a vital role in delivering high-quality, personalized home health care to our clients, helping to improve their health outcomes and quality of life within the comfort of their own homes.

Requirements

  • Valid and current LPN license in the state of Virginia.
  • Proven experience in home health care or a similar nursing role, demonstrating strong clinical skills and the ability to work independently.
  • Excellent communication and interpersonal skills, with the ability to build trusting relationships with patients and their families.
  • Strong organizational skills and attention to detail to manage patient documentation and coordinate care effectively.
  • Compassionate attitude and dedication to providing patient-centered care that respects the individual’s dignity and needs.
  • Ability to travel locally to the patient's home or school within the Richmond area reliably and punctually.
  • Basic computer proficiency for maintaining electronic health records and other documentation.
  • Valid Drivers License

Responsibilities

  • Provide direct nursing care in a school setting
  • Monitor patients’ health status, including vital signs, wound care, medication administration, and other treatments as prescribed.
  • Document patient progress and communicate any changes or concerns promptly to the supervising registered nurse and healthcare team.
  • Educate patients and their families about health management, medication routines, and disease prevention to empower them in their care.
  • Coordinate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive and holistic patient care.
  • Maintain accurate and up-to-date patient records in compliance with company policies, state regulations, and HIPAA guidelines.
