About The Position

This is a travel assignment for a Licensed Practical Nurse (LPN) in Long-Term Care, located in Utica, NY. The assignment is for 13 weeks, starting on 09/01/2026, with guaranteed hours of 37.5 per week. The shift is from 2:30 PM to 10:30 PM (evening). The setting is a Skilled Nursing Facility, specifically a long-term care unit with 40 beds. Common diagnoses on the unit include MS, ortho, and dementia/memory care.

Requirements

• Active LPN license in New York; NURSYS verification of active licenses
• Current BLS certification
• 1+ year of experience in a long-term care (LTC) facility; LTC experience required
• EMR experience with PointClickCare
• Driver’s license with address matching demographics
• RTO (return-to-office) must be included at time of submission
• Ability to work the 2:30 PM–10:30 PM shift; 37.5 hours per week
• Local candidates within 75 miles may be considered at local rate
• Unit specifics: LTC unit with up to 40 beds; common diagnoses include MS, ortho, dementia/memory care
• Scrub color: navy blue (as dictated by unit policy)

Responsibilities

• Administer prescribed medications and treatments per physician orders and facility policy
• Monitor residents’ conditions, vital signs, and response to therapies; document changes in the electronic medical record
• Perform routine nursing assessments; identify and escalate clinical concerns to the supervising RN
• Assist residents with activities of daily living; support mobility and safety, including fall prevention
• Collaborate with the interdisciplinary team to coordinate care and communicate with families as appropriate
• Float within the same building as needed to coverage requirements and unit needs
