About The Position

This is a travel Licensed Practical Nurse (LPN) assignment in Utica, New York, for a 13-week duration. The role is in a Skilled Nursing Facility setting, with a shift of 5x8 Nights. The position offers competitive weekly pay and guaranteed hours.

Requirements

Active New York Licensed Practical Nurse (LPN) license; Nursys verification of active license.
Current Basic Life Support (BLS) certification.
Minimum of 1 year of experience in a long-term care (LTC) facility.
EMR experience with PointClickCare.
Driver’s license with address matching demographics.
RTO (time off) request must be included at the time of submission.

Responsibilities

Provide direct nursing care to residents in a long-term care setting, including monitoring health status and responding to patient needs.
Administer medications and treatments as prescribed, verify orders, and document administration accurately.
Monitor vital signs, observe for changes in condition, and report findings to the supervising nurse or physician.
Document assessments, care plans, and interventions in the PointClickCare EMR system; update as resident status evolves.
Assist residents with activities of daily living (ADLs), mobility, and personal care; support safety and fall prevention.
Collaborate with the interdisciplinary team to implement care plans and participate in rounds and care conferences as assigned.
Maintain infection control and safety protocols; respond to emergencies and initiate appropriate escalation procedures.
