About The Position
Community Care, Inc. is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide one-on-one pediatric private duty nursing care for a 4-year-old child in a supportive home setting in Vandergrift, Pennsylvania. The role focuses on personalized, family-centered care and building a meaningful connection with the child and family. This is a full-time overnight position scheduled Thursday through Monday, from 10:00 PM to 6:00 AM. Community Care describes itself as a family-owned home health agency serving local communities since 1987, with services including skilled nursing and therapy visits, as well as adult and pediatric private duty care.
Requirements
- Current LPN or RN license in Pennsylvania.
- Graduate of an accredited nursing program.
- At least 6 months of nursing experience; home health experience is preferred but not required.
- CPR certification.
- Reliable transportation and a valid driver's license.
- A caring heart, strong independence, and a passion for pediatric care.

Responsibilities
- Assist with activities of daily living (ADLs).
- Perform G-tube care and feedings.
- Monitor vital signs.
- Administer medications safely and accurately.
- Communicate clearly and compassionately with the care team and family.
