About The Position

Adoration Private Duty is seeking a dedicated Private Duty Licensed Practical Nurse to provide patient-centered care in Germantown, Tennessee. The role is based from the Cordova, Tennessee office and involves providing one-to-one care to an adult client. The schedule is full-time, Monday through Friday, 8:00 a.m. to 3:00 p.m. G-tube experience is required.

The nurse follows the client’s plan of care, supports the client’s dignity and independence, administers medications and feedings, and provides other care as needed. The role includes documenting care and outcomes, communicating with the client’s family and care team, and ensuring that supplies and medications are available. Requirements

• LPN/LPT/LVN degree from an accredited college of nursing.
• Current unrestricted registration and license in the state of practice.
• Valid driver’s license, acceptable driving record, and proof of car insurance.
• Professional CPR certification.
• G-tube experience.

Nice To Haves

• At least one year of nursing experience is preferred.
• Home health care experience is preferred.

Responsibilities

• Provide one-to-one care according to the client’s plan of care while maintaining the client’s dignity and independence.
• Administer medications and feedings, provide oxygen and ostomy care as needed, and assess and document results in accordance with the plan of care.
• Document nursing actions, care provided, progress toward established goals, and outcomes.
• Communicate with the client’s family, case manager, and physicians; report changes in the client’s medical condition and needs.
• Coordinate or order supplies, medications, and feedings to ensure adequate amounts are available.
• Ensure patient needs are met in accordance with physician instructions.
