About The Position

Adoration Health is seeking a dedicated Private Duty Licensed Practical Nurse to provide patient-centered care to an adult client in Germantown, Tennessee. The role is based through the Cordova, Tennessee office and provides one-to-one care in the client's home, following the client's plan of care while supporting dignity and independence. G-tube experience is required. The schedule is full-time, Monday through Friday, 8:00 a.m. to 3:00 p.m.

Requirements

• LPN/LPT/LVN degree from an accredited college of nursing, with current unrestricted registration and licensure in the state of practice.
• Valid driver's license, acceptable driving record, and proof of car insurance.
• Professional CPR certification.
• G-tube experience.

Nice To Haves

• At least one year of nursing experience.
• Home health care experience.

Responsibilities

• Provide one-to-one care according to the client's plan of care while maintaining the client's dignity and independence.
• Administer medications, feedings, oxygen, and ostomy care as needed under the plan of care, and assess and report results.
• Document nursing actions, care provided, communications, progress, and outcomes for established goals.
• Inform the physician, case manager, and family of changes in the client's medical condition and needs.
• Coordinate or order supplies, medications, and feedings to ensure adequate amounts are available.
• Ensure patient needs are met in accordance with physician instructions.
