About The Position
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN / T-LPN) for a short-term contract assignment in an assisted living setting. The role involves providing direct patient care, assisting with medication administration, and supporting the healthcare team to ensure quality resident care.
Requirements
- Active LPN / T-LPN License required
- BLS/CPR Certification preferred
- Previous experience in assisted living, long-term care, rehabilitation, or skilled nursing preferred
- Strong clinical, organizational, and communication skills required
- Ability to work independently and collaboratively required
- Compassionate and patient-focused approach required
- Flexibility to work scheduled shifts, including weekends if needed required
Responsibilities
- Provide direct nursing care under the supervision of an RN or physician
- Administer medications and treatments as prescribed and monitor patient responses
- Assess, monitor, and document patient conditions, vital signs, and daily progress
- Assist residents with activities of daily living (ADLs), including mobility, hygiene, and comfort care
- Maintain accurate and timely patient records according to facility policies
- Communicate effectively with patients, families, physicians, and interdisciplinary team members
- Observe and promptly report changes in patient condition or concerns
- Maintain a clean, safe, and organized care environment
- Follow infection prevention, safety, and HIPAA guidelines at all times
- Support additional nursing and patient care duties as assigned
