About The Position

Ashton Creek Health and Rehabilitation is seeking a full-time Licensed Practical Nurse (LPN) for 1st and 2nd shifts. This role offers the opportunity to make a difference in the lives of residents by providing daily care, addressing health needs, and ensuring their safety and comfort. The nursing team is dedicated to helping residents reach and maintain their highest functional abilities, and every role is considered important.

Requirements

  • Graduate of an accredited school of nursing.
  • Valid LPN License in good standing according to State and Federal requirements.
  • Thorough understanding of the principles of safe effective nursing practices within the State of Indiana.
  • Basic computer skills, including knowledge of programs such as Excel and Word.
  • Personal integrity and the ability to deal tactfully with residents, family members, visitors, government agencies, and other personnel.
  • Ability to be discrete and protect the integrity of confidential information.
  • Adherence to Corporate Compliance and HIPAA regulations.
  • Knowledge on the use of personal computers, Word and Excel, and working with office machines such as fax and copying machines.

Responsibilities

  • Providing daily care, health needs, safety, and comfort of residents.
  • Performing professional assessments and documentation of residents' health.
  • Assisting with the management of assigned nursing unit staff.
  • Supervising, performing, and providing written/verbal care assignments, routine skills, and procedures to ensure quality of care.
  • Participating in the development and implementation of resident care plans.
  • Maintaining up-to-date nursing care plans, documenting progress, and making recommendations for revisions.
  • Making daily and periodic visits to observe for symptoms, reactions to medication, and/or treatment.
  • Obtaining and documenting vital signs, intake and output, weights, diet supplements and replacements, cultures, etc.
  • Assisting with the orientation and on-going training of staff to assure required skill levels are maintained.
