Requirements

  • Active Licensed Practical Nurse (LPN) License in good standing.
  • Experience in Skilled Nursing or Rehabilitation preferred.
  • A compassionate, detail-oriented, and dedicated professional committed to excellence in patient care.

Responsibilities

  • Assist in developing and implementing individualized treatment plans for residents.
  • Supervise and support CNAs, including participation in their evaluations.
  • Oversee the Falls Program and Pressure Sore Program to ensure resident safety and well-being.
  • Provide essential nursing services and direct patient care.
  • Conduct and document thorough medical evaluations upon admission and throughout treatment.
  • Perform additional duties assigned to support the best possible care for our residents.
