About The Position

This is a 9-week travel assignment for a Licensed Practical Nurse (LPN) in a skilled nursing facility in Franklin, New Hampshire. The role involves providing direct nursing care to residents in a long-term care setting, administering medications and treatments, monitoring residents' conditions, and documenting care. The position requires working night shifts and collaborating with a healthcare team to ensure resident comfort and safety.

Requirements

• Active Licensed Practical Nurse license in New Hampshire
• Ability to work the night shift (18:45–07:15) and commit to a 9-week travel assignment
• Capacity to perform typical LTC nursing duties and follow the plan of care
• Strong communication and teamwork skills, with attention to detail in documentation and safety
• Eligibility to travel and meet assignment requirements for a skilled nursing facility setting

Responsibilities

• Provide direct nursing care to residents in a long-term care setting, under the supervision of the registered nurse and physician
• Administer prescribed medications and treatments in accordance with physician orders and facility policy
• Monitor residents’ vital signs, assess changes in condition, and document findings; communicate with the supervising RN/MD as needed
• Assist with activities of daily living and other routine nursing procedures, including wound care, ostomy care, and routine treatments per plan of care
• Maintain accurate, timely documentation in resident charts and ensure adherence to privacy and HIPAA requirements
• Collaborate with the interdisciplinary team to coordinate care, respond to call bells promptly, and support resident safety and comfort during night shifts
