About The Position

EmpowerMe Wellness is hiring a Licensed Physical Therapist Assistant to provide therapy and wellness services to older adults in senior living communities. The role is part of EmpowerMe 360, an integrated model connecting therapy, wellness, and medical providers to support residents in living stronger, safer, and more independent lives. The position combines clinical therapy with care coordination and wellness check-ins, with a focus on helping residents maintain independence, confidence, and satisfaction.

• Current professional license in the relevant therapy discipline, including PTA.

Responsibilities

• Provide therapy and wellness services for older adults.
• Conduct assessments, create personalized care plans, and deliver evidence-based interventions as part of a collaborative team.
• Support care coordination through wellness check-ins, caregiver communication, and telehealth support.
• Maintain professional and ethical standards in line with company initiatives.
• Contribute to program development and continuous quality improvement.
