About The Position

EmpowerMe Wellness is seeking a Licensed Occupational Therapist to provide therapy and wellness services for older adults through its EmpowerMe 360 integrated care model. The role supports residents in living stronger, safer, and more independent lives through assessments, personalized care plans, evidence-based interventions, care coordination, and wellness check-ins. The therapist collaborates with wellness and medical providers, communicates with caregivers, supports telehealth, and contributes to program development and quality improvement. The position is offered PRN or part-time.

Requirements

• Current professional license in occupational therapy or another listed therapy discipline (PT, PTA, OT, COTA, or SLP).
• Passion for empowering seniors to live their best lives.
• Strong clinical skills and a collaborative spirit.
• Enjoys teamwork, connection, and being visible in the community.

Responsibilities

• Provide occupational therapy and wellness services for older adults.
• Conduct assessments, create personalized care plans, and deliver evidence-based interventions as part of a collaborative team.
• Support care coordination through wellness check-ins, caregiver communication, and telehealth support.
• Maintain professional and ethical standards in line with company initiatives.
• Contribute to program development and continuous quality improvement.
• Help residents maximize independence, confidence, and satisfaction.
