About The Position
EmpowerMe Wellness is hiring a Licensed Occupational Therapist to provide therapy and wellness services for older adults through EmpowerMe 360, an integrated care model connecting therapy, wellness, and medical providers. The therapist helps senior residents live stronger, safer, and more independent lives by building relationships, supporting confidence and independence, and contributing to a collaborative care experience. The role includes assessing residents, creating personalized care plans, delivering evidence-based interventions, supporting care coordination through wellness check-ins and caregiver communication, and providing telehealth support. The therapist also maintains professional and ethical standards, contributes to program development and continuous quality improvement, and helps residents maximize independence, confidence, and satisfaction.
