The Kidney Care Specialist (KCS) supports patients with Chronic Kidney Disease (CKD) and End Stage Renal Disease (ESRD) through early education, care navigation, and ongoing engagement. The position bridges the gap between diagnosis and informed treatment choice by helping patients receive timely, consistent education and support across the kidney care continuum. The role combines field-based physician and community outreach with virtual patient support; responsibilities may be field-based, virtual, or hybrid depending on market needs. The KCS works closely with nephrologists, advanced practice providers, care teams, internal partners, and referring practices to coordinate patient care, support appropriate referrals, address care gaps, and connect patients with resources. The specialist provides education about kidney disease progression, treatment options, including home modalities and transplantation, and self-management strategies. The role also documents interactions and outcomes, tracks engagement and program performance, supports quality and compliance initiatives, and contributes to continuous improvement of education materials and workflows. Field-based work may require extensive local or regional travel, including visits to physician groups and healthcare locations, as well as travel to regional business units, corporate meetings, and on-site training. The position may occasionally require evening and weekend work. It involves prolonged periods of sitting and computer use. No direct supervisory responsibilities are assigned; the specialist works under Kidney Care leadership and collaborates closely with clinical and operational teams.
