The Intensive Care Coordinator (ICC) is a vital member of the Child & Adolescent Community-Based Services team, responsible for providing intensive care coordination to children and adolescents with emotional and behavioral challenges. This role supports youth who are transitioning from residential care or are at risk of out-of-home placement by helping stabilize them within their home and community. The ICC works closely with families, service providers, and community partners to develop individualized care plans rooted in family strengths, needs, and culture. The goal is to build a strong, wraparound system of support that promotes long-term success.
