About The Position
Signature Healthcare at Home is seeking an Intake Coordinator to support its Home Health & Hospice team and provide service to patients. This is a full-time, benefited position. The coordinator manages patient intake, maintains relationships with referral sources, supports regulatory compliance, and helps ensure patients transition seamlessly to home care. The role may also include cross-training in medical records and scheduling. Signature Healthcare at Home is a Medicare-certified Home Health and Hospice Agency serving the greater Portland, Oregon area. Responsibilities
- Manage all aspects of the patient intake process.
- Establish and maintain positive working relationships with current and potential referral sources.
- Ensure compliance with State and Federal referral and intake regulatory requirements.
- Ensure a seamless transition of patients to home care.
- Build and monitor community relationships.
- Potentially cross-train in medical records and scheduling.
