Intake Coordinator - Home Health & Hospice
About The Position
Signature Healthcare at Home is seeking a full-time, benefited Intake Coordinator to support its Home Health & Hospice team and patients. The role is responsible for all aspects of patient intake, building and maintaining relationships with referral sources and the community, and ensuring compliance with State and Federal intake and referral requirements. The coordinator helps ensure a seamless transition of patients to home care. There may be an opportunity to cross-train in medical records and scheduling. Signature Healthcare at Home is a Medicare-certified Home Health and Hospice Agency serving the greater Portland, Oregon area.
