About The Position

The Home Health Intake Coordinator assures that home care services are provided in compliance with PACE and state regulations, while supporting participant satisfaction and retention. The position organizes, coordinates, directs, and evaluates care functions and provides oversight of services delivered in participants’ homes to help them remain as independent as possible. It also provides assessment, coordination, consultation, and supervision of home care services, and serves as a liaison with contractors such as home care agencies and individual providers.

Providence PACE is a Program of All-Inclusive Care for the Elderly that aims to help older adults remain healthy and live in the community through clinics, home visits, and other services. This role travels to support the South Seattle area and other areas as needed.

Requirements

• 2–4 years of experience coordinating, managing, monitoring, and providing community social or health services.
• 2 years of administrative experience in a healthcare setting.
• Driving is necessary for the role; caregivers must comply with state driving laws and requirements and provide proof of a driver license and auto insurance upon request.
