About The Position

CompassionCare Hospice is seeking a licensed LPN or RN to coordinate hospice intake and admissions in the Las Vegas coverage area. The coordinator guides patients and families through the process, reviews referrals and medical records for hospice eligibility, triages incoming referrals, and arranges urgent or same-day admissions when needed. The role includes educating patients and families about hospice services, coordinating care needs and equipment, verifying insurance and authorizations, maintaining documentation, and communicating with physicians, discharge planners, referral sources, and other healthcare professionals. The coordinator also ensures regulatory and compliance steps are followed and provides referral sources with information about hospice criteria and alternatives when appropriate.

The role is based at 7842 West Sahara Ave., Las Vegas, NV 89117, and is scheduled part-time on Saturdays and Sundays for 20 hours per week. CompassionCare Hospice provides hospice services tailored to the needs of patients and families, with a focus on delivering care at home.

Requirements

• Current, unencumbered state licensure as a Registered Nurse or Licensed Practical Nurse.
• At least two years of healthcare experience.

Responsibilities

• Guide patients and families through the hospice intake and admission process with care and clarity.
• Review referrals and medical records to determine whether patients meet hospice eligibility requirements.
• Triage incoming referrals and coordinate urgent or same-day admissions when needed.
• Educate patients and families about hospice services, answer questions, and address concerns.
• Coordinate services, equipment, and care needs with internal teams and external providers.
• Verify insurance, obtain authorizations, and document referral and patient information.
• Communicate with physicians, discharge planners, and other healthcare professionals to support continuity of care.
• Ensure regulatory and compliance steps are followed throughout the intake process.
• Provide expertise on hospice criteria and support referral sources with alternatives when appropriate.
