About The Position

Graham Healthcare Group is hiring a Hospice Intake Coordinator to support patients by partnering with field staff and leaders. The position serves as a first point of customer service for internal and external customers and can be performed virtually. The schedule is Monday through Friday, 8:00 a.m. to 5:00 p.m., for 40 hours per week. Compensation is $19.00–$21.00 per hour, with a stated maximum cap of $21.00.

Requirements

• High school diploma or GED equivalent.
• At least two years of administrative experience in a healthcare environment.
• Reliable internet service and the ability to work from home with minimal disruption to the workday.

• Home health or hospice intake experience.

Responsibilities

• Serve as the first line of customer service for internal and external customers.
• Receive and document patient referrals to hospice services, ensuring compliance with applicable coverage criteria.
• Review referrals for the appropriate care setting and communicate with the care center.
• Gather information to help the clinical team schedule evaluation visits.
• Coordinate with other departments on insurance verification and payer authorization as needed.
• Communicate professionally and follow up with referral sources about documentation needed to complete referrals.
• Communicate and follow up with Business Development about pending admissions as needed or as directed by the Intake Team Lead.
