About The Position

Adoration Hospice is seeking a licensed LPN or RN to serve as a Hospice Intake Coordinator in Nags Head, North Carolina. This role coordinates the hospice intake and admission process from start to finish, helping patients and families transition into hospice care. The coordinator applies clinical judgment to assess eligibility, facilitate timely admissions, and advocate for patient needs while serving as a liaison among families, referral sources, and the care team. The position is scheduled Monday through Friday, 8:00 a.m. to 5:00 p.m.

Requirements

• Current, unencumbered state licensure as a Registered Nurse or Licensed Practical Nurse.
• At least two years of healthcare experience.

Responsibilities

• Guide patients and families through the hospice intake and admission process with care and clarity.
• Review referrals and medical records to determine whether patients meet hospice eligibility criteria.
• Triage incoming referrals and coordinate urgent or same-day admissions when needed.
• Educate patients and families about hospice services, answer questions, and ease concerns.
• Coordinate services, equipment, and care needs with the internal team and external providers.
• Verify insurance, obtain authorizations, and document referral and patient information.
• Communicate with physicians, discharge planners, and other healthcare professionals to support continuity of care.
• Ensure regulatory and compliance steps are followed throughout the intake process.
• Provide guidance on hospice criteria and support referral sources with alternatives when needed.
