About The Position

Tapestry Hospice & Palliative Care is seeking a highly organized, detail-oriented Intake Coordinator for its Calhoun office. The role is one of the first points of contact for patients, families, referral sources, and healthcare partners. The coordinator helps connect patients to care by managing referrals, coordinating communications and admissions logistics, maintaining accurate records, and supporting an efficient admissions process. The position is in person and is intended for candidates residing in or near Calhoun, Georgia.

Requirements

Experience in hospice, home health, healthcare intake, or Medicare-related services.
Administrative or office support experience.
Strong working knowledge of Microsoft Office applications.
Excellent communication and customer service skills.
Strong organizational abilities and attention to detail.
Ability to multitask and prioritize effectively in a fast-paced environment.
Positive attitude and team-first mindset.
Reside in or near the Calhoun, Georgia area.

Responsibilities

Manage incoming patient referrals.
Coordinate communication with hospitals, physicians, facilities, and referral partners.
Maintain accurate referral records and documentation.
Schedule admissions and coordinate logistics.
Collaborate with clinical and administrative teams.
Provide customer service to patients, families, and healthcare partners.
Support efficient and timely admissions processes.
