About The Position

Signify Health, part of CVS Health, is seeking a full-time physician to provide in-home health evaluations for health plan members, primarily across Virginia and other licensed states as business needs require. The role involves travel up to 100% of the time and may require overnight hotel stays, car rentals, and/or flights. Candidates must have primary residency in Virginia and may need to obtain and maintain multiple state licenses. Visits last approximately 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Physicians use an iPad and clinical workflow. They are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen. Virtual visits may also be conducted as needed. The role includes providing Signify Health services and Diagnostic & Preventive Services products when indicated, with additional per-completion incentive pay for in-home visits and other reimbursement opportunities for completed services. The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. Physicians should be comfortable and able to perform their duties safely in these settings. The role offers a base rate of $110 per hour, with actual pay and other reimbursement opportunities subject to applicable factors and details provided during the application process.

Requirements

Active, unrestricted physician license(s) in the coverage area(s).
Ability to obtain and maintain multistate licenses in coverage areas as required by business needs.
Ability to cover all counties within Virginia and flex to other states as business needs require.
Primary residency in Virginia.
Ability to travel up to 100% of the time; travel may involve overnight hotel stays, car rentals, and/or flights.
Ability to comfortably and safely perform duties in varied home environments, which may include pets, secondhand smoke, and other airborne allergens or irritants.

Responsibilities

Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
Perform additional diagnostic tests when ordered by a member’s health plan, such as diabetic eye exams or spirometry.
Use an iPad and the clinical workflow for each evaluation.
Travel locally and among licensed states, up to 100% of the time, as coverage needs require.
Conduct virtual visits as needed.
Provide Signify Health services and Diagnostic & Preventive Services products as indicated.
Build trusted relationships with members and help connect them with care suited to their needs.
