About The Position

Signify Health, part of CVS Health, is seeking a per diem physician to provide in-home health evaluations and help bridge gaps in care. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the person’s health plan. Physicians use an iPad and a clinical workflow. They are not required to prescribe medication, order lab tests, or change a person’s current treatment regimen. The role may also include virtual visits and locally or multi-state travel as needed. The primary workplace is members’ private residences, where conditions may include household pets, secondhand smoke, and other airborne allergens or irritants.

Per diem employees receive an hourly base rate plus an additional incentive structure, malpractice coverage during Signify Health-related activities, supplies, and other perks. The position is part-time, with anticipated weekly hours of 12. The listed pay range is $83.69–$180.25 per hour. Benefit eligibility depends on hours worked and other eligibility requirements; benefits may include dental and vision coverage, wellness resources, employee discounts, access to certain voluntary benefits, and other programs.

Requirements

• Active, unrestricted physician license(s) in the coverage area(s).
• Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic and preventive services when indicated, such as diabetic eye exams, spirometry, and bone density screening.
• Use an iPad and the clinical workflow to complete evaluations.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Provide Signify Health services as indicated and help connect people with care suited to their needs.
