About The Position

Signify Health, part of CVS Health, is seeking a per diem physician to provide In-Home Health Evaluations to health plan members in their homes and, as needed, virtually. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Physicians use an iPad and a clinical workflow and are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen. The role focuses on building trusted relationships, identifying health needs and care gaps, and connecting members with appropriate care. The primary workplace is members’ private residences, which may include household pets, secondhand smoke, and other airborne allergens or irritants. Local travel and travel within licensed states may be needed. The position is part-time, per diem, with an anticipated 12 hours per week. The stated base pay range is $105.00–$105.00 per hour.

Requirements

• Hold an active, unrestricted physician license in the applicable coverage area(s).
• Be able to safely and comfortably perform duties in members’ homes, which may contain household pets, secondhand smoke, or other airborne allergens or irritants.

Responsibilities

• Conduct In-Home Health Evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic tests when ordered by a member’s health plan, including services such as spirometry, diabetic eye exams, and bone density screening.
• Use an iPad and the clinical workflow for each evaluation.
• Help connect members to appropriate care and address gaps in care.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
