About The Position

Signify Health, part of CVS Health, is seeking a full-time physician to provide in-home health evaluations and help bridge gaps in care. Physicians conduct visits of about 45 minutes in health plan members’ homes, using the time to connect one-on-one and answer health questions. Evaluations include medication and medical history reviews and a physical evaluation, and may include additional diagnostic tests if ordered by the member’s health plan. Physicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or change a person’s current treatment regimen. The role is part of a national network of clinicians working to improve health outcomes and connect people with care suited to their needs. Physicians may travel locally and within licensed states, conduct virtual visits as needed, and provide Signify Health services, including diagnostic and preventive services. The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, or other airborne allergens or irritants. Candidates must be able to perform duties comfortably and safely in these settings. The position is full time, with 40 anticipated weekly hours. The stated typical pay range is $125.00–$125.00.

Requirements

Active, unrestricted medical license(s) in the coverage area(s).
Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, or other airborne allergens or irritants.

Responsibilities

Conduct approximately 45-minute in-home health evaluations and engage with members one-on-one.
Review members’ medications and medical histories and perform a physical evaluation.
Perform additional diagnostic tests when ordered by a member’s health plan, such as diabetic eye exams or spirometry.
Use an iPad and the clinical workflow for each evaluation.
Travel locally and within licensed states as needed.

Provide Signify Health services as indicated, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening.
Help connect members with appropriate care and support improved health outcomes.
