In-Home Physician Full Time - Kalamazoo

About The Position

Signify Health, part of CVS Health, is seeking a full-time physician to provide in-home health evaluations in Kalamazoo. Physicians meet with people in their homes to build trusted relationships, help bridge gaps in care, and connect individuals with care suited to their needs. Visits last about 45 minutes and include a medication and medical history review and a physical evaluation, with additional diagnostic tests when ordered by the person’s health plan. Physicians use an iPad and a clinical workflow. The role does not require prescribing medication, ordering lab tests, or changing a person’s current treatment regimen. Virtual visits may also be conducted as needed. The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. The position requires flexibility to travel locally and within licensed states as needed. Full-time employees are eligible for a comprehensive benefits package.

Requirements

Active, unrestricted physician license(s) in the coverage area(s).
Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, or other airborne allergens or irritants.

Responsibilities

Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
Perform additional diagnostic and preventive services as indicated, including tests such as spirometry, diabetic eye exams, and bone density screenings.
Use an iPad and the clinical workflow for evaluations.
Conduct virtual visits as needed.
Travel locally and within licensed states as needed.
Provide Signify Health services in accordance with the requirements of the role.
Help connect people with appropriate care and support improved health outcomes.
